HAP's Tiered Formulary

HAP's commercial plans use a six-tier formulary structure:

  • Tier 2 (Generic and Select Brand Drugs) — slightly higher cost-share; generic and select brand drugs meeting quality and cost-effectiveness guidelines
  • Tier 3 (Preferred Brand) — preferred brand-name drugs
  • Tier 4 (Non-Preferred Brand and Generic) — non-preferred drugs at higher cost-share
  • Tier 5 (Preferred Specialty Drugs) — complex/chronic disease medications requiring prior authorization and specialty pharmacy dispensing

HAP's 90-Day Prescription Program

HAP's 90-Day Prescription program allows coverage of up to a 90-day supply for medications on the HAP Maintenance Drug List at a participating retail pharmacy. Most major chains and many independent pharmacies are in the HAP 90-Day program. Typically the charge for a 90-day supply of medications on the Maintenance Drug List is two copays.

Prior Authorizations Under HAP Plans

HAP requires prior authorization for specialty medications and certain other drugs on its formulary. This means once you reach $2,100 in out-of-pocket drug costs, your plan covers 100% of covered medications for the rest of the plan year.
